Scarborough Health Network (SHN) and our Scarborough Ontario Health Team (SOHT) and local partners have come together to rollout the COVID-19 vaccine across our community. We are doing this through vaccine clinic sites and mobile vaccine teams. We are pleased to be keeping our community safe and healthy.

Pop-ups

Calling all community members! Bring your family, friends and loved ones to roll up their sleeves at our three pop-ups this Vax Weekend! We’ll be offering Pfizer and Moderna boosters for those 18+ years and paediatric doses for children ages 5 to 11. Plus, we’ll have 1st and 2nd doses for those 12+ who aren’t fully vaccinated.

Clinic information
Boosters (3rd dose)
- Pfizer for those 18+ years old
- Moderna for those 30+ years old
- Must have received last dose 3 months ago
Kids 5-11 (1st & 2nd dose)
- Parent/Guardian must accompany child
- Child must not have received any other vaccine in the last 14 days
- For 2nd doses: Must have received 1st dose at least 21 days ago and have parental consent
Ages 12+ (1st & 2nd dose)
- Pfizer for those 12+ years old
- Moderna for those 18+ years old
- For 2ND dose: Must have received 1st dose of Pfizer 21+ days ago or Moderna 28+ days ago
1st & 2nd dose (12+), Boosters (18+):
- Please bring the vaccination receipt for your last dose, if you are able (download a copy from the Ministry’s COVID-19 Vaccination website)
- If your first vaccination dose or doses was with a non-Health Canada approved vaccine, please bring proof of your vaccination status
- You must bring a form of ID to receive the vaccine (such as drivers license, passport, PR card)
- No OHIP/health card? No problem!
Kids 5-11:
- Must provide a form of photo ID with your child’s name and birthdate (such as a health card or government-issued ID, if you have one)

- Other eligible groups include: Healthcare Workers and designated Essential Caregivers in Congregate Settings, First Nations/Inuit/Métis adults & their household members, those fully vaccinated with AstraZeneca or Janssen, and Immunocompromised
